ADHD Follow-Up Visits

If your child is currently taking medication prescribed for the treatment of ADHD, please fill out our ADHD Parent Survey. In addition, the Vanderbilt Survey (Follow-Up Assessment Survey) should be completed by the parents and at least one teacher as well.

Please bring your child’s most recent report card, as well as any neuropsych testing or IEP plans that may be new to us.
