Patient Dashboard

The Patient Dashboard is a parent-created form designed to help us better serve your child with special needs. This is a one-page summary of your child's medical history that will be placed in the front of the chart. It has proven to save time on office visits, as well as decrease medical errors. The Patient Dashboard includes a Problem List (for example: autism, sleep disturbance, frequent ear infections, fearful of examinations), Medication List (that we will update at each visit), Specialist List, and other pertinent details of the family and social history.

Please take a few moments to download and complete a Patient Dashboard for your child. This form can be returned to us by e-mail, e-mail at your child's next visit, or in the mail.
